Liability Waiver Form
Please Note: This liability waiver form is included as part of the registration process. When you pay for your event, you will have to acknowledge acceptance of this waiver as part. I include it here for convenience.
This agreement releases Jennifer Falkowski, Artist LLC, from all liability relating to injuries that may occur at the any event she runs. By signing this agreement, I agree to hold Jennifer Falkowski, Artist LLC, entirely free from any liability, including financial responsibility for injuries incurred.
I also acknowledge the risks involved by participating in this event. These include, but are not limited to workshop and optional activities. I certify that I am participating voluntarily, and that all risks have been made clear to me.
Additionally, I do not have any physical or mental conditions that will increase my likelihood of experiencing injuries while engaging in the activities included in any event involving Jennifer Falkowski, Artist LLC.
By signing below I forfeit all right to bring a suit against Jennifer Falkowski, Artist LLC for any reason. I will also make every effort to obey safety precautions as listed in writing in the retreat “Terms & Conditions,” and FAQ sections of JenniferFalkowski.com, and as explained to me verbally. I accept responsibility for asking for clarification when needed.
By participating in any event by Jennifer Falkowski, Artist LLC, I grant Jennifer Falkowski, Artist LLC, its representatives and volunteers the right to take photographs of me and my participation. I authorize Jennifer Falkowski, Artist LLC, to use such photographs for any lawful purpose, including for publicity, illustration, advertising and web content.
I encourage participants to take pictures! Participants are welcome to share photos of their experience on their favorite social media channels. I ask that you use the following hashtag: #JenniferDyes
